You were sent formal notices regarding this mandatory inspection and /or required corrective work order to avoid incurring substantial surcharges to your Utility Bill Our records indicate that you have either not yet responded to these notices or complied with their related repair requirements. On November 23 December 24 February 7, 2011, February 24 April 12 April 28 June 7 July 12 and July 28 you were sent the first "Notice of Surcharge to Utility Bill" informing you that a surcharge was being applied to your account on the last Thursday of each month and every month since asking you to contact us. You have now accrued 10 months of surcharges totaling $1,500 00.
