Where there is SAC due, based on the City's good faith in SAC matters, we have allowed use of the rate at the time it should've been paid. There are four items to be resolved. Building permit #81830, dated February 1, 2008, was issued for Graham Construction !' $1,825 p Corporate Office located at 2995 Lone Oak Circle #A without SAC being charged. There is /0- 1 SAC due at the 2008 SAC rate of $1,825 /unit. (See SAC determination letter 101004A3.) / wO Building permit #81903, dated February 14, 2008, was issued for Fitness Together (now I.; $1,825 Sports Specific Training & Rehab) located at 1965 Cliff Lake Road #103 without SAC being V.r j° charged. There is 1 SAC due and again the 2008 rate may be applied. (See SAC Cer determination letter 101004A2.) 3,, Building permit #92414, dated December 28, 2009 was issued for Wagging Tails Pet $4,000 Resort located at 3275 Sun Drive. Information submitted to MCES for SAC determination letter 091218B2 stated that grooming may be added at a later time. Information discovered during the current SAC Review found that grooming is available. There are 2 SAC due at the 2009 SAC rate of $2,000 /unit. (See SAC determination letter 101004A4.) 4. Building permit #93477, dated May 13, 2010, was issued for a single family home located/ $2,100 at 570 Caylin Court without SAC being submitted. There is 1 SAC due at the 2010 SAC rate of $2,100 /unit. Environmental Services Please thank Peggy Fleck for all of her assistance during the review. Total Amount Due: $9,750 To correct these items, please remit the amount of $9,750 on the current month's SAC report on the adjustment line. If you wish to dispute these findings, please provide additional information within 60 days of this letter. The vacancy date, which is different than the demolition permit issue date, is required on all SAC -D forms; and • Our SAC credit rules have changed. Keith Olson Wagging Tails Pet Resort 612 - 369 -2912 City of Eagan recently did a on -site inspection of our grooming room and confirmed we have only 1 tub and no plumbing or rough in for a 2nd tub. Our current plans do not include a 2nd tub at this location. Information provided to MCES for determination 091218B2 in December 2009 stated pet grooming may be added at a later date. Information discovered during the current SAC Community Review shows that grooming is available. Based on the 2 grooming tubs, as shown on the building plan, there are 2 SAC units due.
