Well and Boring Sealing Record H313109. Operations Management Facilities Management On June 26, 2013, you called me about a well sealing you had completed at the location Fleet Management above. The well in question supplied water to the clubhouse at Parkview Golf Course. The Parks well is on property owned now by D R Horton, Inc. At the time of sealing you listed Scott Transportation Carlston as the owner. At the time of our hone conversation you told me you had sent a Highways p Y Y Surveyor's Office notification for the well sealing to the Minnesota Department of Health Well Transit Office Management Unit (MDH), not realizing that the well was in Dakota County, a delegated well program area. Dakota County requires a permit application and permit review prior to the start of well construction or sealing activities. A permit application was subsequently received on June 26th, 2013. Please be advised that future well construction or sealing activities conducted without a permit may result in a triple permit fee. On June 27, 2013, 1 inspected the sealed well. At the time of my inspection I noted the presence of what appeared to be high solids bentonite grout on the ground around the 4- inch diameter casing. Measurement at that time found bentonite grout approximately 6- feet down from grade in the well casing. Dakota County Environmental Resources received the Well and Boring Sealing Record on July 1, 2013. According to the Well and Boring Sealing Record (Record) you sealed the well on June 18, 2013. 1 checked with the MDH to determine if they had received notification for sealing, they said they had not. On June 28th you e-mailed to me a response letter from the MDH to you indicating the well in question was located in Dakota County, that letter was dated June 21, 2013. The unprocessed well sealing notification and check were ea Printed on recycled paper with 30% post-consumer waste. AN EQUAL OPPORTUNITY EMPLOYER returned, with the letter, to you by the MDH. Because the MDH did not process the notification, there was no record in their system when I called to check. The Record indicates the well was 365-feet deep and was cased to a depth of 318-feet. In the Geological Material section of the Record, you indicate there are unconsolidated sediments from 0 feet to 318 feet and limestone from 318 feet to 365 feet. The well casing extends to 318 and is open hole from there to 360 feet This is consistent with the stratigraphy from the Well and Boring Record for Unique Well Number 207283, an irrigation well used for the golf course. In the Grouting Materials Section of the Record, the grouting material is listed as Quik grout from 365 feet to 0 feet and that 13 bags were used. MN Rules, part 4725.3850 requires that the portion of the well completed in bedrock must be sealed with neat-cement grout or cement-sand grout. According to the sealing record and my observation at the time of inspection, the well sealing was not completed in compliance with well code requirements. To bring this well into compliance, you must remove the high solids bentonite grout from the open hole portion of the well in the Prairie du Chien limestone and properly grout the open hole portion of the well with neat-cement or cement-sand grout. The portion of the well above the bedrock formation may be grouted with a grout approved for use in unconsolidated materials. All work must be completed with a Dakota County Well Inspector present. Please contact me a minimum of two full work days prior to commencing work on site. Work must be completed during normal business hours. If you believe our information is incorrect or incomplete, please contact me within ten business days. This well sealing must be corrected within 30 days of receipt of this letter.
