This form is intended for temporary outdoor COVID-19 testing and/or vaccination stations utilizing temporary structures, tents, canopies for drive -up or drive -through facilities to administer COON testing to clients. If no site plan is on file, applicant may utilize online maps to illustrate the below information. Initial Requirement Location of the tent, trailer or temporary structure. Include dimensional information from buildings or roperty lines. A minimum of five (5) car lengths (approximately 100 to 120 feet) should be provided behind each testing lane. Illustrate the general traffic flow of vehicles, including points of entry and exiting. /AziTo - ensure pedestrian and customer safety, the owner will utilize traffic cones, ropes, signage or other barriers to direct traffic to appropriate waiting or queueing areas. Traffic queueing shall not interfere with any pedestrian walkways or ADA accessible routes. Traffic queueing shall not block designated fire access drives. No portion of the outdoor testing area or traffic queueing shall utilize public rights -of -way (sidewalks/trails, boulevard or streets). No accessible (ADA) parking spaces may be converted into outdoor use areas. If determined by the Public Works Director, a traffic management plan may be required if traffic conditions, real or perceived, negatively impact public streets; this may include hiring of traffic management services. I understand you are on board with the proposed drive-thru COVID testing set-up on the Emagine Theater site. We have been working with Tim Cushing on this and have advised that for an outdoor temporary use like this we do need confirmation of the fee owner's permission to set up this operation. An email will suffice, but it has to come from the fee owner of the property and at present, that is still Spirit Master Funding X, LLC. I understand both of you have already contacted the Spirit folks in Texas, and thus far, they have not responded. Once you have the written authorization from them to proceed, we'll be happy to review the proposal and turn that around as quickly as possible. We are ok with this. We are giving him authorization to set up in the section directly to the east of the building. This is the lot that will be subdivided to create the final out lot. Thanks, Dale Haider Northwoods Entertainment, 20653 Keokuk Avenue, Lakeville, MN 55044 --- Cell 612-986-7436 2 Rapid RX Testing LLC — Site Plan & General Description of Operation Rapid RX Testing LLC will be providing COVID Testing to the general public at 2055 Cliff Road, Eagan MN 55122. We were issued a CLIA Certificate 24D2200503 (from the Minnesota Department of Health to perform these tests which are FDA approved for EAU (Emergency Use Authorization). The site operation hours will be from gam-8pm seven days per week. Weekend hours may vary. The site will be staffed with 2-4 people along with one site supervisor. All staff will be trained according to the manufacturer's instructions prior testing any individuals. Staff will be equipped with the proper PPE attire while performing these services and all waste will be properly disposed of into a biohazard waste container. These containers will be disposed of through Stericycle waste services. We will also have a Maintenance person on our staff that will maintain the general area. Our site plan (Attached) shows traffic coming in from Cliff Lake Road into the East parking lot where we will split traffic into 2 lanes using cones. The 2 lanes will then lead to the tent area where the actual tests will be administered. Once the test has been completed the cars will then go to the parking area indicated on the site plan to wait for the results. Once they have the results they can exit 3 different ways out of the parking area. All cars coming into the area will be instructed to stay in there vehicle throughout the entire testing process. Scheduling for tests will be done through our website in 10 minute intervals. The site which is t still under development is going to go live the week of December 14h . We have also bought marketing spots on KQRS that will begin that same week. You can text COVID to 68683 and it will take you directly to our site. All scheduled tests will be paid for ahead of time prior to people showing up. We do anticipate that there will be people who show up without having scheduled first.
