Applicant/Permitee: Signature Issued By: Signature STORMWATER MANAGEMENT PLAN REQUIREMENTS ■ Depict current property layout using recent overhead imagery(e.g., Dakota County GIS, Google maps, etc.)or drawing; ■ Depict proposed de-icer location(s) upon the property; o De-icer storage location requirements: ■ At least 25 ft from any storm drain inlet ■ At least 50 ft from the high-water level of any water body or ditch that conveys stormwater runoff ■ At least 100 ft from the high-water level of any impaired water body • Allow sufficient access to any manholes,water supply valves, or other nearby utility structures ■ Must be screened from view or placed in rear of property, if possibly ■ Must be placed on an impervious surface(e.g. concrete or bituminous pad) ■ Will not receive direct or indirect stormwater runoff(e.g. roof downspouts, gutters, snow-pile melt, etc.).
