Traffic shall be allowed to pass and to be protected at all times. If it is not possible to allow traffic to pass, a suitable detour must be provided and plans submitted to the Director of Public Works 72 hours in advance. 2. Barricades shall be erected in a manner which will provide suitable visibility in all directions. All barricades shall be in good condition, and all signs shall be of such size and legibility to provide adequate warning to oncoming traffic. At least two 7" flashing amber lights shall be mounted on each end barricade with one on an advance warning sign. 3. Excavations must be shored or sheeted when necessary to prevent under-mining of roadway, trailways, utilities, or for safety reasons. 4. Guys or stays shall not be attached to trees on right-of-way or pxivate property without written permission. 5. Flagnen shall be furnished by the party or organization performing the work whenever the work being done creates a hazard either to the traffic using said road or the personnel engaged in the construction, or when directed to do so by the City. OPERATIONS 1, Permit on Job--Permits or copies shall be kept on the site of the work while it is in progress in the custody of the individual in charge, and shall be exhibited upon request made by any City official. 2. Provisions and Specifications--These general provisions, specifications and Std. Plate P-1 shall be considered as forming an integral part of each and every permit issued for operations within Eagan. The work suthori2ed by this permit shall be done at such time and in such manner as shall be consistent with the safety of the public and shall conform to all requirements and standards of the City. If at any time it shall be found by the City that the work is not being or has not been properly performed, the permittee, upon being notified by the City, shall immediately take the necessary steps, at his own expense, to place the work in condition to conform to said requirements or standards. 3. Execution--The permittee shall use diligence in the execution of the work authorized under this permit in order not to endanger or unnecessarily obstruct travel along any xoad ox trailway. Operations shall be so conductad at all times as to permit safe and reasonable free travel over the roads and trailways within the limits of the work herein prescribed. All safety measures for the free movement of traffic shall be provided by the permittee at his own cost. 4. Coriformity to Laws--The installation shall be made in conformity with all applicable laws, regulations and codes covering said installations. All installations shall be made in conformity with regulations of governmental agencies for the protection of tlie public. e. The applicant shall furnish a bond or financial guarantee in the amount to be determined by the City which is requixed to ensure adequate & timely completion of repair. This bond or financial guarantee shall remain in effect for.2 years subsequent to completion of street repair to pzotect the City from defects in material, worlmanship or non-compliance with City Standards or specifications. . , Yermit No. Page 3 of 5 b. The applicant shall furnish evidence of puhlic liability insurance of not less than $100,000/300,000 and property damage of not less than $25,000 issued by an insurance company authorized to do business in the State of Minnesota on which the City is named as an additional insured patty. c. Except for the negligent acts of the City, its agents and its employees, the permittee shall assume all liability for, and save the City, its agents and its employees, harmless and defend same at its sole cost and expense fxom any and all claims for damages, actions or causes of action arising out of the work to be done herein and the continuing uses by the permittee, including but not limited to the placing, constructing, and reconstructing, maintatning, protecting and use of said facility under this application and permit for construction. 5. Existing Facilities--The utility facility and installations shall not interfere with any existing utility facility on the City's right-of-way or easements. It is the responsibility of the applicant to call for necessary locations of existing utilities. (Gopher One 454-0002) 6. Private Property--The work permit or permit for construction as issued does not in any way imply an easement on private property. 7. Quality of Work--Finished surface, base and sub-base of road or trailway upon completion of work shall be at least equal to or hetter than specifications of originally provided road or trailway in accordance with City Standard Specifications. Surface shall be finished within 48 hours unon comvletion of backfill. 8. Curr_ng Trees--The permission herein granted does not confer upon the permittee the rig?.- to cut, remove or destroy trees or shrubbery within the legal limits of the rig:`.-of-way, easements that are not specifically identified on the plan attached to this permit or relieve permittee from obtaining any consent otherwise required from the owner of the property adjacent thereto. 9. Drainage--All waterways and lines of drainage shall remain operative, 10. Pole Anchors--Poie anchors, anchors, bracea or other construction will be permitted within right-of-way or easements and will be approved on a case by case basis. 11. Driving Limitations. a. Driving or parking on City trails or sidewalks shall only be permitted for those operations requiring direct access to the boulevard area. 6. Vehicles operating or parked within the right-of-way area shall utilize their warning flashers at all times. c. Vehicles driving on trails or sidewalks shall not operate in excess of 5 miles per hour. Vehicles shall operate at slower apeeds vhen weather conditlons, trail conditions, poor visibility, obstructed sightlines or other conditions require special precautions to ensure the safety of trail users and the general public. d. Driving shall not be.pern¢itted within those boulevard.areas where damage to turf trailways or other infrastzucture may occur. e. The purpose of our excavatioa observations was to assist in judging the suitabIlity of the soils in the bottom of the excavations for fill and foundation support. Background Information During our work, we had access to the foundation evaluations performed by our firm (Braun Intertec reports BABX-93-144 and BABX-93-178). The general soil profile in the proposed Huebsch Linen building area was about 3 to 14 feet of fill and topsoil over alluvial deposits of sand. The generaC soil profile in the proposed spec building area was about 1 to 7 feet of fill, possible fill and topsoil over alluvial deposits of sand. Mr. Tom Ryan of R.J. Ryan Construction indicated the Huebsch Linen building will be a one- story, slab-on-grade structure with a building footprint of approximately 20,000 square feet. Roof loads will be supported on perimeter tilt-up walls and interior columns. Mr. Ryan also indicaterl the spec commercial building will also be a one-story, slah-on-grade suucmre with a building footprint of approximately 15,000 squaze feet. Roof loads will be supported on perimeter tiit-up walls and interior columns. Locations and Elevations The Iocations and elevations used in the field at the time of our work and referenced in this report were related to stakes placed by others. Braun Intertec does not do construction surveying and must rely on the accuracy of staking placed by the surveyor or the contractor. Elevations and locations should be considered approximate. a. R. Ryan Construction Project BABX-93-276 September 9, 1993 Page 2 Fxcavation Observations Huebsch Linen Building Our excavation observations were performed between May 20, 1993 and June 3, 1993, by a project engineer. We met Mr. Bob Nyan of Nyan Excavating at the site. Our services included observing the soils encountered in the bottom of ihe mass eaccavation for the building pad and the trenches dug for the perimeter footings. In addition, shallow hand auger borings were performed in the soils exposed in the botbom of ihe excavation. Soil classifications were determined by examining the auger cuttings. Approximate density of the soils encountered was estimated by judging the force required to advance the auger. The elevation at the bottom of the footing trenches ranged from approximately 877 in the loading dock azea to 881. Several hand auger probes were performed to a depth of about 2 1/2 feet in the bottom of the excavations. These hand auger probes encountered brown and dazk grey sIlty sand, sand with silt and poorly graded sand with varying amounts of gravel. These soils were judged to be in a medium dense condition. Based on the site staking, it appeared that the mass excavation had a suitable 1:1 oversize. This oversize is necessary for lateral suppoR of the footings throughout the fill sequence. No oversize was necessary in the footing trenches, because the footings were placed on the natural(y occuning alluvial soils. Spec Commercial Building Our excavation o6servations for the mass excavation of the building pad were performed on May 20, 1993, by a project engineer. Ryan Construction Project BABX-93-276 September 9, 1993 Page 3 We were not notified by the excavator when excavation proceflures were being performed for the strip footings for the spec commercial building. Therefore, no observations of the fooung trenches were performed for the spec commercial buiIding. Compaction Testing Huebsch Linen Building The on-site fill soils were used to fill the excavation and as backfill azound the footings. The fill was compacted with a self-propelled vibratory compactor. Nine compaction tests were performed as the fill was placed. The test results indicate the fill tested met or exceeded the minimum recommended densiry of 98 percent of the standazd Proctor density (ASTM D 698) below footings and 95 percent in other building pad areas. The results of these compaction tests are attached to this report. Spec Commercial Building The on-site fill soils were used to fill the excavation and as backfill azound the foocings. The fill was compacted with a self-propelled vibratory compactor. Fourteen compaction tests were performed in the buIlding pad, under the floor slab and in the footing backfill. The test results indicate the fill tested met or exceeded the minimum recommended density of 98 percent of tfie standazd Proctor densiry (ASTM D 698) below the footings and 95 percent in the remaining fill and backfill. The results of these compaction tesu are aiso attached to this report. Cancrete Testing During this time period, three sets of concrete cylinders cast by the contractor for the Huebsch Linen building were tested for compressive strength. The results of the tests are attached to this report. To date, all test results indicate that the specified strength was met at the 28-day age. Conclusions Based on the results of our excavation observation, hand auger probes and compaction tests, it is our opinion the soils encommtered in the bottom of the excavations we observed are suitable for support of the anricipated 3,000-pound per squaze foot loadings. General Services performed by Braun Intertec have been conducted in a manner consistent with that level of caze and skill ordinazily exercised by members of the profession currendy practicing in this azea under similaz budget and time resuainu.
