Pursuant to your request, I ani providing a leiter to serve as a certificate of compliance for the
manufacture and installation of the precast hollowcore planks which were installed at this
location.
The plank were engineered to support a concrete topping and a live load of 75 ;psf uniform car a
2000 lb. point load as the International Building Code requires. The plank were pinned with a
mbar dowel at a minimum 4'-C" on center through the plank and a minimum 4' into the bond
beam. The plank keyways were grouted according to our instructions and will provide the ability
to distribute vertical loads. to adjacent planks. Selection oi' water proof membranewe

discretion of the owner.
Section 1704.2.2 f the code states that special inspections are no required when the work is
done on the premises of a fabricator registered and approved to do such work without special
inspections. To provide documentation of this fact, 1 have enclosed a copy of a letter of
confirmation from the Precast Concrete Institute that Molin Concrete is a member in good
standing of the PC1 Plant Certification Program. You will note that the PCI Plant Certificatio
program is recognized by many code bodies.
The Plant Certification Program requires the maintaining of inspection records for all concrete
pours. These inspections are performed by Molin's Quality control staffand the records are kepi
indefinitely. These records indicate that the precast concrete provided for this project by Molin
Concrete was manufactured in accordance with the contract documents and the approved shop
drawings.

rebar
• Flow through bond beam block was used on each side to accommodate a double core
fill with #5 rebar vertically on each side of opening
• W8X18 steel beam with %"x11" bottom plate installed across opening on south wall.
Each end contains an 8"x 8"x W' plate, double core fill and #4 rebar.
• Vertical core fills 32" on center containing #5 rebar 3" from inside face of block was
