If SEH passes their inspections Linda Dralle will contact us so we can final the bldg permit. 4/30/09 Final CN: Dan Zienty of S.E.H. Met with T- Moble @ O'Leary Park 4/23/09. Dan suggested (per Linda Dralle's e-mail to me dated 4/24/09) that we could issue a final on this permit, but that we should send T- Mobile a letter stating that we were never afforded the chance to perform the required footing inspection. To that end, I have contacted Chris Miller from T- Mobile and asked that he get the City a signed letter from the engineer of record stating that the footing for the monopole in question was constructed per his engineered drawings. This may be easier said than done.*** 6/15/09 Final CN: - Apparently there were no on site inspections prerfomed by the engineer of record. I have asked T- Mobile's representitive, Chris Miller, to send us a signed affidavit from the forman in charge of construction, stating that the footing/foundation and pole were built and or installed per the engineered drawings reviewed by the City of Eagan Inspections Dept. This letter was received and inserted in the parcel file. In the future the City will asked that a Special inspection and testing schedule be required and filled out prior to issuing a permit for these cellular towers. This will require that the (independent) special inspector be on site for footing and final inspections. This should insure that if a mistake is made (as in this case) and we are not called to perform the required inspections, we will have the Special Inspector's documentation to fall back on. It should be noted that (per the adopted Building Code) the City inspections have been a requirement and will always be a requirement.
