AddiCion No. 1, East of PiloC Knob Road un Deerwood Drive. It was noted that
Che cost of services ware at $660.00 accoxding to the contract coasiderably

Craig Osweli, of Ulteig Engineers, Inc./ J. H. Dahlmeier Engineering, Inc., visited the site for a
special inspection of the Polysteel insulated concrete form wall construction for the residence
located at 1300 Deerwood Drive in Eagan, MN. Construction is to be done in accordance with
design drawings created by J. H. Dahimeier Engineering, Inc. dated 8-22-01 and revised on 1-
08-02. The following specific items were noted:
1. At the time of the site visit, the basement level wall construction was complete. No
upper level floor or wall framing was in place. The vertical and horizontal reinforcing
was in place in accordance with the construction documents. The stoop area hooks had
yet to be placed. The walis were being shored with Polysteel steel shoring posts.
2. The ICFLF floor hangers were in place at the bearing walis in accordance with the
design drawings.
3. Joe Wohlford of Rocco Altobelli asked if the ICFLF anchors could be spaced further
apart at the non-bearing walls. Oswell informed him that the spacing could be increased
to 48" on center at detail S7 (non-bearing walls not supporting grade) and to 24" on
center at detail S7A (non-nearina walls supoorting qrade). Oswell also stated that the
floor bracing is to be installed in line with the ICFLF anchors. Wohlford stated that he
would relay this information to the contractor.
4. The strip footing at the interior wall between "Rocco's area" and the basement was
placed incorrectly. The contractor intends to pour an addition to the strip footing along
side beneath the correct wall location. The contractor has installed #4 dowels at 24° on
center and a horizontai #4 bar tied to these doweis in order to tie the footings together.
The size and reinforcing of this footing correction is structurally adequate.

Site Inspection Report
The contractor informed Oswell that the window openings at the rear near the patio and
the first floor man door were a few inches wider than planned. In addition, the concrete
columns 6etween the openings (8" by 3.25") are smaller than planned. As result, the
lintel is spanning approximately 17 feet over all three openings with the two concrete
columns between them. Oswell checked the calculations and determined that the lintel
is adequate as constructed, using the columns as support points. However, the
concrete columns are not adequate. The columns are to be widened to a minimum of 6"
x 8", otherwise steel posts are required (as directed in note 4 on sheet S10). The steel
tubes are to be a minimum of 3 x 3 x'/:' tubes or 3.5" diameter schedule 40 pipes. The
steel columns are to be attached to the lintel bottom and the window sill in accordance
with detail 510.
The lintel between the house basement and the area marked "Rocco's Area" on the
architectural drawings does not have enough concrete above the opening to use the
concrete lintel design. The lintel is to be wood framed using either a(2) 2 x 12 SPF or
(3) 2 x 10 SPF header. The header is to bear a minimum of 4" in a treated wood pockei
at the concrete wall on each end. The bottom chord bearing floor trusses are to bear on
top of this header.
Joe Wohlford of Rocco Altobelli informed Oswell that at Rocco's area, a mechanical
opening through the interior Polysteel wall would be required. The opening would be
directly against the front exterior wall and would measure 48" wide by 12" high. This
opening is acceptable. It is to be framed over with the same header described in item 7
above. At this location, the trusses may need to be face mounted with a truss hanger in
order to achieve the desired opening size.
9. At the stairwell, behind the elevator, a wood beam is required to support the top of the
stair frame. The contractor wishes to use a double 12" LVL. This beam size is
structuraily adequate. The beam may be attached to the ledger on the elevator shaft
and on the exterior wall, using Simpson face or top mount hangers sized for the header.
10. The brick ledge installation was in progress at the front of the residence. Installation
was in accordance with the design documents.
11. The floor siab in the 6asement was not installed at the time of the site visit.
12. Oswell informed the contractor that the dowels at the stoop to the basement wall can be
drilled in after the wall is poured or hooked up into the above grade wall depending on
how the elevations lined up. The dowels are to be embedded a minimum of 12" into the
concrete wall.
13. No other significant discrepancies were noted.
