Eagan, MN 55122 Dear Mr. Avdulov, Thank you for the cooperation that was extended to me by you during my recent inspection of your facilities. Our goal of fire prevention in our community relies on teamwork between our inspection staff and the business owners in the community. As you know, we requested Minnesota Energy to also stop by when we were at your facility on Friday 11-30-07. The technician checked for any carbon monoxide readings in the fresh air area and also in the furnace room. All readings were in the low/safe area at this time. He also checked for any natural gas readings/leaks on the furnace pipes. There were some small leaks found on some of the pipe connections on the furnace, but nothing in the dangerous area at this time. We did notice heavy corrosion on the bottom of the vent pipe for the exhaust. I highly advise that you have a professional HVAC technician/contractor to thoroughly inspect your heating system at this time. As for the inspection of your church, multiple Fire and Building Code violations were found. I did have Eagan Building Inspector Craig Novaczyk accompany me on my second visit on Monday 12-3- 07. We were not able to find any building permits ever issued for the construction that was done in the` lower level of the church. That is an issue that needs to be corrected and addressed as soon as possible with the permit and building inspections division with the City of Eagan. What I found from a Fire Inspection side were violations from small, no address numbers on the Diffley Rd. side of the building to extension cords being used for permanent wiring to some very large issues like missing and non- existent Exit and Emergency lighting, non-working Exit door missing any door hardware on the northwest comer of the building, and the absence of an approved Fire Alarm system. This does not include all violations. It is due to these findings after our inspections that we ask that you and your church council or church board members hire a certified Architect to provide a comprehensive code analysis and set of plans showing that the building is compliant with Fire and Building codes. We ask that this plan be presented to us within the next 30 days and the violations be corrected in the next 60 days with the exception of the non-working Exit door on the northwest side of the building, which must be repaired in the next 14 days. During the course of that walk through, I discovered numerous Building Code violations. It was evident that there was a considerable amount of construction performed to create the daycare facility and Sunday school classrooms that currently exist in your church building. After looking into it further, we found that no building permits were taken out for the work that was done. A letter dated 12/04/07 was sent to you by Darrin Bramwell concerning these matters. He asked that you provide a comprehensive code analysis and set of plans drawn by an architect certified with the State of Minnesota, showing what you propose to do to bring the daycare facility and Sunday school classrooms into compliance with Building and Fire Codes. There was a deadline of 30 days attached to this request. To date, we have not received any proposals from you or anyone else affiliated with the Grace Slavic Church. Eagan, MN 55122 Dear Mr. Avdulov, Thank you for the cooperation that was extended to me by you during my recent inspection of your facilities. Our goal of fire prevention in our community relies on teamwork between our inspection staff and the business owners in the community. As you know, we requested Minnesota Energy to also stop by when we were at your facility on Friday 11-30-07. The technician checked for any carbon monoxide readings in the fresh air area and also in the furnace room. All readings were in the low/safe area at this time. He also checked for any natural gas readings/leaks on the furnace pipes. There were some small leaks found on some of the pipe connections on the furnace, but nothing in the dangerous area at this time. We did notice heavy corrosion on the bottom of the vent pipe for the exhaust. I highly advise that you have a professional HVAC technician/contractor to thoroughly inspect your heating system at this time. As for the inspection of your church, multiple Fire and Building Code violations were found. I did have Eagan Building Inspector Craig Novaczyk accompany me on my second visit on Monday 12-3- 07. We were not able to find any building permits ever issued for the construction that was done in the lower level of the church. That is an issue that needs to be corrected and addressed as soon as possible with the permit and building inspections division with the City of Eagan. What I found from a Fire Inspection side were violations from small, no address numbers on the Diffley Rd. side of the building to extension cords being used for permanent wiring to some very large issues like missing and non- existent Exit and Emergency lighting, non-working Exit door missing any door hardware on the northwest corner of the building, and the absence of an approved Fire Alarm system. This does not include all violations. It is due to these findings after our inspections that we ask that you and your church council or church board members hire a certified Architect to provide a comprehensive code analysis and set of plans showing that the building is compliant with Fire and Building codes. We ask that this plan be presented to us within the next 30 days and the violations be corrected in the next 60 days with the exception of the non-working Exit door on the northwest side of the building, which must be repaired in the next 14 days. Was there any correspondence between the church and the City of Eagan at the time of the take over of the Cedar Grove Utilities System by the City of Eagan or at the time that the church connected to the Cedar Grove System originally that you can find? 4. What type of assessments or connection charges have been paid by the utility system including those a) that were paid originally and b) at the time the addition to the church was put on. At the time of the addition as I recall, there were some conditions that were placed on paying assessments at that time and any correspondence should be researched in that respect. 5. I will need a breakdown on how the calculations were made on the assess- ments including any connection charges and special assessment for water, sewer, etc. Further, the applicable assessment year should be submitted.. The Council adopted the recommendation of the Assessment Committee to provide that the assessment for Public Facility type use on the corner be based on the long side for lateral purposes less the street allowance and that the short side be given a credit of double the normal 150 feet corner lot allowance for a total of 300 feet or the length of the short side whichever is less. This would mean that the water lateral assessments would be assessed as follows: County Road #30 - 559.20 feet 33.00 feet for street allowance 526.20 feet There would then be no assessment on the Rahn Road side. It is my understanding that the Church Board has approved the proposal and has agreed to the dismissal.
