work authorized-hereunder and shall be notified ot its completion within fivz days thereafter. The notice of permit issued by the
Commissioner shall be kept securely posted in a conspicuous place at the site of operations. ..
4. No chan9e shall be made, without wNtten permission previousty o6tained froin the Commissioner of Natural Resources, in the hydraulic
dimensions, capacity or location of any items of work authorized hereunder.
5. The permittee shall grant access to the site at alt reasona6le times during and after consiruction to authorized representatives of the
Commissioner of Namral Resources for inspection of the work authorized hereunder.
6. Tbis Permit may be term+nated by the Commissioner of Natural Resources, without notice, at any time he deems it necessary for the

After checking on the septic system ai 578 Miclugan Court, it was determined that this
property is still not on City sewer.
Chariie Borash checked the manhole and could not see any connections coming into the
manhole. At his request, I called the Mahoney's and informed them that there were some
differing opinions as to whether they were on City sewer. Mrs. Mahoney informed me
that they had the lines run when they put in the water, but as of this time have not hooked
up to City sewer.

Mechanical.
Currently my home has a well and septic system. The well,
although deep, is very slow. It is on2y marginal for household
use. The position of my lot makes it almost impossible to either
dig a new well or service the existing one. The only practical
option is city sewer and water.
I hope the applications were completed in a satisfactory

Dear Mr. Mahonev:
We recently received a pernrit application requesting authorization to repair an existing timber abutment at your
dtiveway crossing a portion ofHay Lake in Eagan. I had met with a contrador at the site in November to inspect
the site and discuss pernut requirements for the replacement of the timbera with a block wall.
Before the application is considered complete and can proceed, I will need to have the following items addressed:
1. please provide gound level photographs of the existing condition
2. what are the approximate dimensions of the work proposed (length and height)7
3. provide a copy of the engineering or other technical analysis for the proposed block wall
4. provide a one to two-paragraph written description of the project
5. an application fee in the amouat of $150 payable to the Minnesota Depaztment ofNatural Resources
I will hold your application in a pending status until such time I receive a reply from you. Most of the items
required above were mentioned as being needed for inclusion in the application when I met with the contractor
last fall. Work cannot commence until authorization from t7ils office is given.
