Enclosed, please find the new application and a check for 90.00 dollars.
I need your assistance with coordinating the inspection date and time with Aztecs manager and crew leader. They will
be there to assist with removing and replacing any of the areas required for the inspection.
Aztec is required to pull a new permit, due to the existing permit is expired.
There is not a scope of work; due to the roof was installed in the year 2000- The inspection is to verify that the work
performed in 2000, was to code.
If you have any questions, please give me a call. Please contact me as soon as the permit has been issued and you are
ready for the inspection. Please allow at least 3-5 days lead way, so that we may have the time to coordinate around
everyone's schedule.

Very soon, we will be issuing the re-roofing permit that you applied for @ 4470 Oak Chase Way, Eagan Minnesota.
We will require that you provide the following information:
1. Identify the shingle type (wood shingles, shakes, etc.)
2. Supply the roof pitches
3. Provide the shingle manufactures installation requirements if available
4. Indicate the attic area in square footage and how you are going to meet the requirements for ventilation per the Code
It will be up to you as the contractor to coordinate the inspections with The City of Eagan's Building Inspections Department.
You will also have to provide safe access for the inspections that are subject to approval by the inspector at the time of inspection.

I spoke with Dale, and he has indicated that the City of Eagan would be willing to perform
the required inspections if Aztec Roofing pulls a new permit for the Gaspar re-roof. Of
course, this would most likely require them to remove shingles to verify compliance with
the Code.

Dear Mr. Sendecky,
I had contacted you earlier this year regarding the problem with the
construction company, Aztec Roofing, located in Burnsville MA7. I
received back a letter stating that a file was opened in your department for an
investigation of my complaint.
2 have been working with my lawyer on this matter and we had an inspection done and the
problems at hand are much, much worse than original thought. It is very clear that Aztec
had clear intent to defraud, and further concealed things by not having my property
inspected as per the City of Eagan requirements. It turns out that most of the items that
were agreed upon by contract (and pad for) were simply not done. The $30,000 roof contract for a cedar shake roof of my home in Eagan that was damaged by a
storm in 2000. Aztec stated they would replace shakes, install ridge vent, ice shield,
chimney flashings, step flashings, other vents, gutters, and pull a permit. Within 90
days of them claiming the job was finished I experienced leaks in my garage area. Several
months later I started complaining about the missing ridge vent and this went on non-stop
for several years without anything being done. 2001 thru 2003 I filed complaints with the
MN AG and the BBB. 2004 thru 2007 I continued to file complaints and contact Aztec but
they simply stopped responding. Now they claim the roof is out of warranty despite the
fact that they never did the work as per contract or code.
Aztec did not install the shakes properly. The gaps were too large and too many were
misaligned. 2 also discovered shakes not even stapled in, instead they were just shoved
under other shakes. In some areas the shakes did not extend past the roof enough and
others showed too much of the shake was exposed. .
I paid for a continuous ridge vent, this is extremely important due to the vault of my
house design. Aztec did not install this, despite my numerous complaints (for years) and
even filings with the BBB and the NIIN AG. This has been the core issue for my year of

trying to get them to finish the job properly - the lack of ventilation caused my house to
become very warm in the summers and build up ice in the attic space in the winters which
has caused tens of thousands of dollars worth of damage to my house's interior.
Aztec promised to install (as per contract) 678 linear feet of ice shield. Aztec installed
only about 90 feet which is well below code even though they were paid to install the full
amount. In some areas we even found the ice shield to be set back as much as 3" from the
soffit.
I paid for sidewall step flashing ($1300.00). Again, Aztec lied and simply reused the old
step flashing. This can easily be proven as the old flashing had paint on it from previous
house colors going back for decades. I have 2 chimneys and Aztec was paid to install new flashing during the roof replacement.
They just pried up the old stuff and then re-stapled it down. Again... easily proven by
old paint splatters over the past decades. I ended up having leaks behind one of the
chimneys because of this.
I also paid for 3 additional vents for the areas that were not included with the ridge
vent that was supposed to be installed. Aztec installed some vents, but in the wrong
areas. Even with that, they left some of the old vents still installed. Again, simply re-
using rather than replacing. '
The company was paid toinstall new gutters, but they did not even install a drip edge,
which is vital with cedar shakes. This caused water to run behind my soffit and fascia,
causing major rot. Also, I had issues with water even running down the interior wa11s and
even water coming out of outlets!
Aztec's failure to install proper venting caused major ice buildup in my attic spacing and
thus over $20,000 worth of damage to my interior.
My insurance company, Liberty Mutual, stated the damage to my interior was due to
'contractdr error' and thus not covered. Aztec tried to get away with all of this by not
having ANY of their work inspected by the city. They did pull a permit, but according to
the City of Eagan the job was never started. A quick inspection by the chief engineer
shows several basic violations.
And tonight, just to add salt into the would, I received a"cold call"
from Aztec Roofing, stating there was hail damage in my neighborhood and they are offering
a free inspection for a free roof. Other roofing companies refer to Aztec Roofing as just
a bunch of storm chasers... I wish I had known this at the time I chose them to destroy my
home.
The
interesting about sales call from Aztec Roofing is that 2 am on the Federal Do Not Call
list, of which they must adhere to if it's been more than 18 months since 2 have done
business with the company. This sales call was left on my answering machine and is
available in a computer audio file if needed.
1008 of the items I paid Aztec to do was either done wrong, or they simply lied for
financial gain. This was clearly 'intent to defraud'.
I have plenty of pictures if anyone is interested. I also have letters from the City of
Eagan stating no inspections were done, letters from another roofer showing some of the
defects, and even lots of letters from the MN Attorney General's office - as they too
tried to get Aztec to respond'to my requests for them to fix things. Even my lawyer has a
hard time getting responses. They just choose to ignore any problems.
This is not the first issue against Aztec Roofing by unhappy customers.
I chose Aztec because they told me they used nails on shakes and 'local people'. When the
crew showed up NONE OF THEM SPOKE ENGLISH and all of the license plates were from Texas
and Oklahoma. After the first day they stopped using nails and switched to staples.
Aztec was also contracted to install 3 skylights. The crew was so clueless they installed
2 of the 3 skylights with the roof trusses running through the middle of the window! They
fixed it, but the skylights were crooked and the flashing was installed wrong, so it looks
horrible.

"FOrgetting" to install something or slipping up on one phase of a project can be
attributed to sloppy work, but when every phase of a contract was not done and they
claimed it was, well.., that pretty well shows what their intention was. And never
calling up the city inspectors at any time during the roofing installation proves it even
more.
Aztec also has a history within this state. I understand they are also currently being
sued by someone else in Surnsville.

- please see page 18 that shows that the owner of the company was fined by the city of
Minneapolis for failing to properly obtain permits and was instructed to resolve customer
complaints. This company has also operated under several other names, including Alcove
Roofing, USA Roofing Inc., Andrews Roofing LLC.

customer who was forced to take legal action.
Z understand there are criminal charges that can be brought against a contractor such as
theft by false representation under Minn. Stat.
609.52, and failure to use proceeds paid for improvements to real estate
under Minn. Stat. 514.02. Aztec Roofing cut corners and lied about
products used and services paid for, so they could increase their profit margin. I would
like to file a criminal complaint in this matter.
This company has a history of bad business in the state of Minnesota and their defiance of
simple building codes and requirements is a danger to anyone who fa11s for their false
claims. Speaking of false claims:
Their website even claims they are a"Charter Memher of the Cedar Shake and Shingle
Bureau". well, I contacted the folks at cedarbureau.com and received the following
response:
"Thank you for your e-mail. The companies you are asking about in the State of Illinois
or Minnesota have never been members of our organization. Therefore we cannot comment on
this specific company.,,
My lawyer, another contractor, and a rep from Aztec Roofing (along with their lawyer Andy
Moran) met with me at my house on October lst to review the damages and claims. Aztec .
made no comments as to why they did not install or provide any of the products or services
they were paid to do. They did not comment as to why no inspections were ever done. And
everyone agreed that the water damage to the interior of my home was due to improper
ventilation, bad flashing, and failure to install drip edges. 100& were items Aztec
promised to do and lied about doing it. I had blueprints and photos showing the original
construction of the home (including the original ridge vent system) and Aztec could not
dispute any of my charges, but again, they simply ignore the problems.

I wanted to thank you for the information and also for digging out the necessary papenvork regazding Aztec Roofmg's permit
on my residence at 4470 Oak Chase Way. I am now writing to Liberty Mutual regazding the facts they were charged for
services that were not done, as well as the Departmen[ of Commerce, requesting an investigation.
Also, I was wondering if you were able to get in contact at all with Aztec... they still aze not returning my telephone calls. Is
there any way that I can file a complaint with the City of Eagan against them as well?
The more that I dig into this, the worse it appears and that company should not be allowed to ruin any more people's houses.
I just diswvered yesterday a source of some of my leaks... appazently when they replaced by roof and gutters they did not
install any flashing between the gutter and the fascia, so water runs down in into my attic spaces there - which has not caused
rotting of the wood in several of these areas.
Are there any type of codes regarding gutter installation on this matter?
I am very concemed about the quality of what I cannot see, as there were no inspections done for this permit. Some of the
evidence that shows this company had no clue what they were doing was items like the roof peak shaked (where the ridge
vent is supposed to be) originally were not uniform... they had some going one direction and other pointed in the opposite
direction. Another indication was when they replaced my skylights - the workers actually cut the hole right over a rafter and
installed the skylight with the rafter running down the middle!

to the revised stonn sewer assessmEnt roll as discussed at the Deceinber 1, 1981
Council Nleeting. The City Council received your written request to reschedule your
oontinued public hearing due to your inability tA attend the January 12, 1982 con-
tinued public hearing. Based on your written recTuest, the City Council will pro-
vide an opporttmity for you to appear at the Febniazy 2, 1982 regular Council meet-
ing to malce your verbal presentation eoncerning your objections t,o these assessc!ients.
Please be advised that in confoxmazce with the forn± letter sent to,your attention
concerning these nrojects as a result of the Januaxy 12th public hearing, written
ocimrients and additional information must be sulanitted to the City Clerk no later
than February 1, 1982. Although your verbal presentation will be on Febntasy 2,
this is being providecl for through special oonsideration and the deadline for sub-
mission of written material cannot be extended due to the time frams required to
oontinue with this assessment appeal process.
In smnury, if you wish to submit written inforniation pertaining tn your objections
for the stozm sewer assessnent, they must be sutanittecl to the City Clerk prior to
Febnuary 1, 1982. If you wish to rialce a personal presentation to the City Council
at the Febnaarv 2, 1982 meeting, you must sulxnit a caritten request to be on the
City Council agenda to my attention no later than Jannary 27, 1982. This will allaa
you to be scheduled as a part of the official aqenda for the FPbruary 2nd meeting.
All items for Council amsideration that are not part of the official agenda are
